Vestibular Migraine North End

The ICHD-3 states that vestibular migraine diagnosis requires:

1. A minimum of five episodes
2. A history of migraine vestibular symptoms, such as vertigo or dizziness, lasting five minutes to seventy-two hours.
3. At least half of the episodes had a migraine headache or other symptoms related to migraines.
The duration of vertigo bouts or “dizzy spells” may vary from person to person, as indicated by the diagnostic criteria. Many patients indicate that their vertigo bouts persist for hours, but others say they might linger for minutes or days, and a small percentage say they just endure for seconds.
It could be necessary to rule out other vestibular diseases. It is advised that a variety of physicians, including general practitioners, neuro-otologists, neurologists, and ENT specialists, handle this.
In individuals with suspected vestibular migraine, the vestibular function tests should demonstrate that vestibular function is within normal bounds. Meniere’s disease and other vestibular disorders should be suspected when vestibular function tests yield abnormal results.

Options for Vestibular Migraine Treatment

Similar to other migraine kinds, vestibular migraine is treated with typical migraine preventive drugs such amitriptyline, candesartan, propranolol, and flunarizine. Flunarizine is frequently the preferred preventative in this situation and is available via headache centers but not from general practitioners. In this situation, blocks of the greater occipital nerve may also be utilized.
The acute treatment for headache attacks is the same as what is typically advised for migraines. This depends on non-specific pharmaceuticals like paracetamol and non-steroidal anti-inflammatory drugs like naproxen and ibuprofen, or migraine-specific treatments like triptans. Avoiding opioids is advised. A brief course of prochlorperazine may be helpful as a possible vestibular sedative for vertigo attacks, and antiemetic drugs such domperidone and ondansetron may also be helpful.
